Service Description
Community Support helps people living with mental health or substance-use challenges become more stable, independent, and connected to their community. Our Community Support Workers meet with individuals in their homes and communities to provide practical, one-on-one guidance. We can help with everyday skills such as managing a household, budgeting, taking medications as directed, handling stress, building healthy relationships, and working toward education or employment goals. We also help individuals connect with healthcare, behavioral health treatment, housing, transportation, employment services, and other helpful community resources. During difficult situations, we provide support, help follow crisis and relapse-prevention plans, and coordinate with the individual’s treatment team. Every person receives support based on their individual needs and recovery goals. Our goal is to help people build the skills, confidence, and support system they need to maintain their well-being and live successfully in the community.
